Health Insurance
Consolidated Appropriations Act Requirements: Transparency in Coverage regulations require our health plan provider, Anthem, to publish their in-network negotiated rates and out-of-network allowed amounts in the form of machine-readable files (MRF) made available on a public website. The regulations, part of the 2021 Consolidated Appropriations Act (CAA). To access this information, please take the following steps:
- View Machine Readable Files on Athem's website
- Please enter the following Employer Identification Number – 54-6001292 under the “Find Files for an Organization” section.
- If you have problems with the files, please contact Anthem directly.
Your Rights and Protections Against Surprise Medical Bills: When you get emergency care or are treated by an out-of-network provider at an in-network hospital or ambulatory surgical center, you are protected from balance billing. In these cases, you should not be charged more than your plan’s copayments, coinsurance and/or deductible.

ACA Reporting Guidance - 1095-C Availability and Information
As part of the Affordable Care Act (ACA) we provide current and previously covered individuals
with a 1095-C form as a record of the health insurance coverage they and their family members
were offered and enrolled in, if applicable. 1095-C forms are available through the FCPS
Employee Self Service (ESS) payroll portal.
How to Access Your 1095-C Form(s) in ESS:
- Login to ESS
- Click on Pay/Tax Information
- Click on 1095-C
- The most recent tax year is selected by default (2024).
- To save/print your form, click the View 1095-C Image link in the top right hand corner
IRS regulations have determined that employers are no longer required to automatically furnish
paper 1095-C forms. Current and previously covered individuals have the right to request a
copy of the statement. The request may be made by:
