Medication Administration Guidelines

Parents/Guardians are advised to give medications at home whenever possible. If it is necessary that a medication be given during school hours, the following guidelines must be followed:

  • All medication must be brought to school in a manufacturer-labeled, unexpired container/bottle, with appropriate label intact. Physician prescribed medication must be in the original pharmacy labeled container with the student's name and medication dosing information.
  • A student who must take a physician prescribed medication for a period of one month or more must provide the school with a completed Request and Authorization/Parent Consent for Administering Long-Term Prescription Medication During School Hours.
  • A student who must take a non-prescriptive medication or a physician prescribed medication for a short period of time should complete an OTC/Short Term Medication Form. Any exceptions to the manufacturer's recommendation or the physician's prescription will require an additional note from the student's physician.
