New Sports Physical Form
Beginning July 1, 2025, all student-athletes must submit a new Preparticipation Physical Evaluation (PPE) Form before participating in any school-sponsored athletic activity. This form must:
- Be fully completed and signed by a licensed healthcare provider
- Confirm that the student has been found physically fit for athletic participation
- Be dated within 14 months of the healthcare provider’s signature
- Be submitted to the school principal or designee along with parent/guardian consent
Completed physical forms must be submitted directly to the student's school. Forms should not be brought or emailed to the School Board Office.

Q: On July 1, 2025, if a student has a physical signed within the last 14 months, is another physical needed?
A: No. If the student’s physical was completed and signed within the last 14 months, it remains valid.
Q: If the valid physical was done before July 1, 2025, using the old form, does the student need a new one?
A: No. Physicals completed before July 1, 2025, using the old form, remain valid for 14 months from the date signed.
Q: When does my student's current physical form expire?
A: The student's current physical form expires 14 months from the date it was signed. If a form’s expiration date falls on a day that does not exist (e.g., September 31 or February 30), the expiration will default to the last valid day of that month.
