New Sports Physical Form

VHSL Serving Youth Since 1913

The Virginia High School League (VHSL) has released a new Preparticipation Physical Evaluation (PPE) Form. Beginning July 1, 2025, all physical examinations must be documented using the new VHSL PPE form. 

This form must be:

  • Fully completed and signed by a licensed healthcare provider.
  • Confirm the student is physically fit for athletic participation.
  • Dated within 14 months of the healthcare provider’s signature.
  • Submitted to the student’s school.

Any physical completed on or after July 1, 2025, must be submitted on the new form. Physicals submitted on the previous form will remain valid for 12 months from the date of the exam, but will not be accepted for exams dated July 1, 2025, or later.

We encourage families to check the expiration date of their student-athlete’s current physical and ensure the new VHSL PPE form is used for their next examination.

FAQs

Q: On July 1, 2025, if a student has a physical signed within the last 14 months, is another physical needed?
A: No. If the student’s physical was completed and signed within the last 14 months, it remains valid.

Q: If the valid physical was done before July 1, 2025, using the old form, does the student need a new one?
A: No. Physicals completed before July 1, 2025, using the old form, remain valid for 14 months from the date signed.

Q: When does my student's current physical form expire?
A: The student's current physical form expires 14 months from the date it was signed. If a form’s expiration date falls on a day that does not exist (e.g., September 31 or February 30), the expiration will default to the last valid day of that month.
